Individualized Therapy Care for Recovery
UM Health-Sparrow provides you with leading edge, effective and personalized physical, occupational and speech therapy for patients of all ages at 11 convenient rehabilitation locations. Our teams are devoted to helping you achieve optimal long-term outcomes and independence with all aspects of daily living. Your recovery from surgery, injury or other physical dysfunctions or impairment will benefit from services provided by our multidisciplinary rehabilitation teams.
Scheduling Appointments
For therapy services in Carson, Charlotte, and Ionia, call the local location listed below. All other inquiries, please call Central Scheduling at 517-364-5255.
Please bring the following pieces of information when scheduling an initial appointment:
- Prescription information
- Insurance card(s)
- If Auto or Workers Compensation case
- Case manager phone #
- Employer
- Authorization or claim #
- Date of injury and location
- Please have your calendar available to determine the best available appointment time for you.
We will do our best to reschedule your missed appointments. Due to patient scheduling, please arrive on time for your appointment so we can provide you with the full treatment and time. Please bring any forms, authorizations, identification, insurance information, and diagnostic reports that may be necessary for your visit with us.
Prescriptions for therapy
By Michigan law, we must have a current prescription that has been written within 30 days of your initial visit for occupational, physical, or speech therapy. The prescription should include diagnosis, frequency, and duration of treatment. The prescription must be from a physician (M.D./D.O.), physician’s assistant, or dentist. Bring your picture ID so we can make a copy of it.
Cancellations
We request a 24-hour notice from patients who find it necessary to cancel a scheduled appointment with one of our physicians or therapists. If you cancel and/or do not show for more than three appointments in a row, it is within the provider’s discretion that you may be discharged from services.
Insurance Questions
To inquire about insurance participation, surgery benefits, co-pays, and deductible, please contact your insurance carrier. For other insurance questions, our team would be happy to assist you.
