Sparrow Financial Assistance Program (SFAP) Summary
SFAP is the name of UMH Sparrow’s financial assistance program. It covers medically necessary services performed by at UMH Sparrow facilities and services performed by UMH Sparrow providers. SFAP may be available to you if you are:
- Uninsured
- Insured, but cannot afford your out-of-pocket costs
- Insured, but receiving services that are not a benefit of your insurance plan
Eligibility for SFAP is determined by:
- Income and assets*
- Michigan residence
- Medical Necessity
*Patients having services at a designated Rural Health site have a separate SFAP application process. Feel free to access our Rural Health application information.

The UM Health-Sparrow Financial Assistance policy does not apply to emergency and medically necessary care provided by non-employed providers or groups who provide care at UMH Sparrow sites; e.g. non-covered providers. While many non-covered providers or groups may recognize UMH Sparrow’s criteria and apply similar discounts, their financial assistance policies are independent. A list of UM Health-Sparrow covered and non-covered providers is updated quarterly and maintained. A paper version of this list may be obtained free of charge by calling 833-227-7664.

After UM Health-Sparrow receives the completed application and required supporting documentation, a final determination will be communicated to you.
In addition to SFAP, UM Health-Sparrow Financial Counselors can assist uninsured and underinsured patients in applying for Medicaid, ACA Marketplace coverage, and other programs to cover medical costs.
