Community Health Needs Assessments

What is it?

A Community Health Needs Assessment (CHNA) is a community-driven data collection effort to identify the strengths, challenges, health priorities, and opportunities to address and improve public health.

Community Health Needs Assessments are important to collect local information on the needs of the community and give the people in the county an opportunity to be heard. It is a resource that guides decision making for many organizations and helps determine the root cause of certain health issues.

University of Michigan Health-Sparrow is proud to partner with Healthy! Capital Counties along with local health departments, other healthcare systems and local community-based organizations to conduct a CHNA every 3 years for the tri-county region.

Community Health Improvement Plan

After the survey is completed, the data is analyzed and top health priorities are determined. From these priorities, organizations create an improvement plan showcasing their goals on they can make a difference in helping the community.
