Sparrow Care Network and Clinical Integration

In 2014, UM Health-Sparrow and physician leaders initiated the creation of Sparrow Care Network (SCN), a Clinically Integrated Organization.  Through this partnership, SCN physicians and UM Health-Sparrow are transforming the way we deliver health care to our patients and region.  SCN is led by physicians and is comprised of primary care, medical, surgical and hospital-based specialties.  It includes both independent, as well as UM Health-Sparrow employed physicians, working together on a collaborative basis to develop and implement a new care delivery model.  Today, the network is more than 600 providers strong throughout mid-Michigan and spans a range of health care needs.

Clinical Integration is the most effective response to the current shift from fee-for-service to value-based reimbursement.  Nationally, the best physicians and health care systems are achieving Clinical Integration.

SCN Bulletin

The SCN Bulletin offers the latest information from Sparrow Care Network. Full versions are emailed directly to SCN members, but highlights can be found here each quarter.

Choosing Wisely: Evidence-Based Recommendations

The mission of Choosing Wisely is to promote conversations between clinicians and patients by helping patients choose care that is:

  • Supported by evidence
  • Not duplicative of other tests or procedures already received
  • Free from harm
  • Truly necessary

Beginning in 2012, national organizations representing medical specialists have asked their members to identify tests or procedures commonly used in their field whose necessity should be questioned and discussed. This call to action has resulted in specialty-specific lists of  “Things Providers and Patients Should Question.”

To help patients engage their health care provider in these conversations, and empower them to ask questions about what tests and procedures are right for them, patient-friendly materials were created based on the specialty societies’ lists of recommendations of tests and treatments that may be unnecessary.

Success Stories detail ways in which clinicians are implementing the campaign in their practice and how patients are avoiding unnecessary care.

The SCN Board of Managers was created to establish and oversee the vision and mission of SCN through the management of operations, policies, procedures, and payer relationships.
